I would put a comma only where there would normally be a new line and the Suite should go with the street.

But maybe you don't use the Queen's English

I would spell out Boulevard if at all possible, because the period comma thing is ugly. But the Suite comes before the street.

I believe it can be very useful to avoid obfuscation, incertitude, and dubiety.

I've come around to the position that it doesn't hurt. I don't like it, think it introduces weird pauses, but if your writing is sloppy enough to need it, or some other circumstance requires one, then you need to be consistent and use it always.
