As long as the Radio Paradise app is installed on your phone you should be able to select it from the Android Auto apps menu.

If you want to use voice commands to start Radio Paradise,  then yes,  it will only launch it on TuneIn,  so you would need to have that installed on your phone.

From searching around the interweb, it appears that there is a way to play the RP Android App in Android Auto.  But I can't find information on how to implement it.  When I tried to start RP, Android Auto says Tunein Radio isn't installed.  Is this the only way to get it to work?  I'm more than willing to put in the research and work to get it done if somebody can point me in the right direction.  Thanks.
